Prevea Health and Phytel Announce Care Management Conference for Patient Centered Medical Homes

GREEN BAY, WI and DALLAS, TX — (Marketwired) — 08/14/13 — and today introduced the , the premier industry event designed specifically to deliver critical intelligence, resources, and best practices for care managers and professionals involved in care coordination within patient centered medical homes (PCMH).
